This is a pretty controversial issue. We're advised in school to have a
backup plan if applying for a competitive specialty. The program directors however, prefer candidates with a dedication. With often more than ten candidates for each available position, it's understandable that the program won't treat you seriously if they know this is not your only choice.
The key is to avoid cross references among departments.
1) Don't apply for categorical IM and preliminary medicine at the same
program, because preliminary medicine is designed for other advanced
2) For some small programs in a hospital, multiple programs may be handled
by the same program coordinator. He or she may easily find out the same
candidate for more than one specialties there;
3) If each program has its own program coordinator, most likely you may get
away of it (just take the risk if you have to);
4) If possible, try to apply different specialties at different places. For
example, apply for a competitive specialty at lower-tier places and the
backup (less competitive) at higher-tier places.
