Summary for externship with Dr. He's program

It was a difficult decision for me to have an externship in New York
this summer. I had to leave work without payment, and I am also the
only person earning money for my family. However, I believe it was
very important for me, a truly old CMG, to have the special training
from Dr. He, a medical education specialist in US. I came to the
externship without any hesitation. When I took the flight back to
Michigan, I was very happy that I made a right decision.

The externship started from participating in 3-days Diagnostic
Pathology Course organized by Chinese American Pathologists
Association (CAPA). This educational course provided latest diagnostic
development in the fields of surgical pathology and cytopathology,
with a format of didactic lecture and case-based presentation on
diagnostically challenging cases. It was a great chance to learn from
national pathology experts and practicing anatomic pathologists about
diagnostic immunohistochemistry in different subspecialties. The
course will be very beneficial to my pathology residency application
and future practice in pathology.

Then I shadowed Dr. He for daily sign outs of various GI cases. I
appreciate Dr. He was a very patient teacher. He taught me the steps
of locating pathologic changes under microscope, such as going through
whole section in low magnification, then check the details in higher
magnification. He showed me normal histologic appearances of
esophagus, stomach, colon and rectum, comparing with pathological
changes. He encouraged me to check the slides and give him my own
diagnosis. After practicing several cases, I could identify some cases
with gastric ulcer with H. pylori infection, which appeared as small
blue dots in the mucosa under Giemsa staining. It was exciting to use
microscope to identify the different histologic appearance of
hyperplastic polyps compared with tubular adenoma in colon, and the
appearance of squamous cell carcinoma compared with esophagus polyps
with dysplasia. In addition to teaching the diagnostic knowledge in
pathology, Dr. He also tested my knowledge about pathology, reviewed
my residency application package and gave me very valuable
suggestions.

I also shadowed Dr. Tan at NPD for several days. Dr. Tan was also a
very good teacher. He clarified some base concepts that confused me
for a long time, such as the definition of intradermal nevus,
junctional nevus and component nevus under the view of microscope. I
went through various cases with Dr. Tan including psoriasis, nail
fungi infection, Melanoma, many GI cases and GYN cases. It was very
impressive to learn from him to use genetic and molecular knowledge,
combining with diagnostic methods to reach an accurate diagnosis.

I also had lots of hands-on chances to process specimen in this
diagnostic pathology center. Every technician could do multiple works
and acted as professional educator. I actively involved in all
activities including processing of gross tissue specimens, analysis of
cytological specimens, microbiology test, immunohistochemistry,
molecular pathology and flow cytometric analysis.

I would say this hands-on externship is very helpful for me to pursue
a career in pathology. The externship greatly consolidated my
understanding about normal histology, surgical pathology and cytology,
increased the knowledge about the process of clinical laboratory
medicine.
