people sent me messages recently regarding how we should prepare for the
coming intern year, here are my several thoughts, cannot type chinese now.
i bought washington manual as lots of others who started their IM intern
year. but frankly, i didn't find it's very useful.first of all, the
information were very brief and superficial, didn't really help much when
you trying to make assessment and plan. also most time the problem is you
don't know what's the problem, eg, if we know it's COPD, it's CHF, it's AMI,
we know what to do, it's just you don't know what's going on, what's your
treating that made you distressed.
my suggestion is, get a good online rescource, Uptodate is stronly
recommended, it's very use friendly, easy access, quick search and very
straight forwad.and you can just input the symptoms and search, it would
come up lots of related information to guide you for further work up.
if your hospital doesn't provide uptodate, get it anyway, 200 dollar every
year i think, and you can get money back since most programs have flexible
MD consult is also a good reference website, if you have time and want to
read more about some disease, MD consult offers lots of online books that
cover different specialties.
at home, i use harrison for reference, and mayo clinic for IM board
and the most important thing is, care about patient first, most your
knowledge comes with the daily care of patients, not from sitting in cubicle
reading articles. don't be greedy and over ambitious,or trying to impress
others by knowing millions rare diseases, your patient care is the best
speaker of your knowledge level.
also keep in mind that, if your attending mentioned some disease, some
guidelines, make sure you read it later, so next time you will know what he
is talking about. attendings tend to repeat stuff they are interested, and
knowing what they are intersted, is the best way to leave him a good

other things you need when you start intern year
--a good internet connection at home, very important
--a good couch where you can relax, cry, call people, eat and read...
--make sure your appartment is in a quiet neighborhood, because your sleep
is very precious
--contact information of other interns in case of emergency (eg. you need
others to cover)
--always keep a clean kit in your cubicle or desk(tooth brush/paste, oral
