First I apologize that I still don't know how to type Chinese.

I started part-time OB experience in Neurology after I finished my CS exam
last April. I was doing clinical research at that time, whenever I didn't
have patients visit in the morning, I would attend the morning rounds. My
boss suggested me to spend more time in Consults, General services and Neuro
ICU because for old graduates like me, doing Neuro Consults provide the
opportunity to expose to a variety of diseases, not only limited to
neurological diseases, spending time in Neuro ICU helps me to learn how to
treat sick patients.

I spent the first 4 weeeks in Consults, most of the weekdays and all the
weekends. The attending that I followed was not impressed by my clinical
skill but he was very impressed by my enthusiasm :) For any specialty,
especially neurology, showing interests is the most important thing for most
attendings. Because I don’t have any neurology background except 3 years
Clinical research, this is extremely crucial to me.

At my spare time, I tried to read as many as I can, forced myself to ask
questions even sometime my questions seemed very silly, prepared my
presentation very well, brought snack to the team during the weekend, being
polite and considerate to everyone, even the nurses and medical students.
It is not a good feeling to be the invisible person, we have to work hard to
become a noticeable teammate. Sometime it depends on good luck, if the
attending and residents are nice, it is a lot of easier.

Because I work in Neurology and those attendings are my colleagues, it has
been a relative pleasant experience for me. I did my OB in neurology off and
on about 3-4 months. At the meantime I was working on my OB opportunity in
MICU and General Medicine. The IM department is kind of arrogant, I didn’t
get the preliminary iv from them, nor did I get the permission to do OB in
MICU. With the help from a few doctors that I work with, I got a month OB
opportunity in General Medicine in February. I was very nervous at the
beginning because I felt I was a “loser”, also because our neurology
program is catergorical, if I get into my home school, I have to do my
internship year here anyway. The pressure was on, but I think I did a good
job overall. It is funny that one of the two attendings I followed in
general medicine is the PD who rejected my preliminary IM application! Other
than all the efforts that I did when I was doing Neuro OB, I followed the
resident and intern very closely during this month IM OB. Whenever my team
was on call (q4), I will spend the whole day with them, sometime with the
resident and sometime with the intern. The best time to learn is when they
admit patients in the ER, the first time when they see patients. The
resident, intern and medical students are very nice people and I am very
lucky to get chance to work with them. I also fed them very well, food,
snack, chocolate, candy… They loved it 

So overall, what I suggest is to spend as long as you can doing OB, be
attentive and enthusiastic, be nice to everyone, being humble but also know
how to impress people from time to time.

I will be taking off for a vacation in a few days, let me know if you have
any questions.
