“How did I fail my residency” in another way, is: what should I have done
in my residency? If I had a time machine, would I do something to fix it?
Let me breakdown the whole topic into: aims, knowledge, colleagues/
attendings/politics, and life. I am not trying to preach and I hate that
the most. So, I intended to stay away from sentiments, cliché or life-
coaching. Just read on if you find them helpful and want to do things
differently. So, what is the aim of residency?
Attitude is everything. It is the fundamental principle in our culture but
unfortunately, it was lost when I transitioned to the medical field. If I
started from scratch again, I would:
Aim high and high—not only to survive but to thrive, this is the most
important thing in residency. The misperception of being miserable in
residency has long history dating back to the book “intern’s blue” in
1980. And the book was an intern’s side-project (they could make a book
during the miserable residency!)
CMGs do get unfair treatment and some of them failed, but the majority
graduate flawlessly. It is not because they have superior intelligence or
they work harder or have better people-skills, it is just simple that the
residency is not difficult as you imagine.
1.Don’t underestimate yourself. Look around and how many FMG got the jump-
start from the residency and started climbing up the ladder? Many FMG chairs
the division, department and serve as high-rank administrative staffs. How
many CMG chairs the department or program? The difference is that CMGs have
the misperception and they aim low just in order to survive, Indians take
the owernship of the residency and the country and therefore they lead.
There will be discrimination that is guaranteed. There will be opportunities
that are also guaranteed.
A.If your aim is to survive the residency, all that you will be doing is to
get hurt by repetitive work, intentional or un-intentional insults and
mockings, extra calls as compared to others…et al. If you aim high, you
will be motivated to overcome all of these shits with happiness and self-
B.If you aim high, you will surpass your peers without noticing it.
C.Make yourself a leader. Teach people, help people, do good things for the
people, stand up and speak for people. And you will feel soon that “
Guanxin” in residency is easy. If you aim to survive, you will constantly
have tension with attending, colleagues, seniors, juniors because being
defensive is the number one skill to survive. By being defensive, you will
get more hostility and insults.
D.There is no fairness between CMG and AMG, period. So, aim higher, be
smarter, don’t get shy in front bosses and make them look up to you. Don’t
just hide in order to survive and comfort yourself “Americans are actually
stupid and I don’t care”.
2.What are the aims?
A.If you want to do private practice, don’t picture yourself sitting there
and numbering how many 100-dolloar bills you earn a day. By setting this as
your aim, all challenges and difficulties in the residency will be
sufferings and hard to swallow.
B.If you want to do academics, start as early as possible. Talk to people,
ask for help, present your ideas, get yourself be parallel to the new star’
s projection, and learn to organize to mutli-task. Get grant done as early
C.If you want to be the physician-scientist, be aware that it is not going
to be 50%-50%. You don’t need to worry about money to have a decent life
but you need to worry about your grants to support your lab. Invest your
time to find out if you are capable of doing it or you are interested in
doing it because if you miss it, you will regret it.
D.Private practice gets you more money, more work, more “boring patients”
that make you serve and get connected to the community , less time to have a
cup of good coffee, while academics gets you more free time, more
interesting/difficult case that justify the value of yourself being a doctor
, less work and less money to cruise the Caribbean. Don’t think that
private makes money 2 times higher or just 2 times higher and don’t think
that academics earn as little as 150,000. Do your own research from the
beginning as early as possible and decide where you want to go
